These school questions, even involving grade school, made me giggle a little: apparently, everything DOES go on your permanent record!

20. Were you in the armed forces. Dates of service, places of service.

Personal Characteristics

21. Describe your predominant emotional characteristics prior to the marriage.

22. What kinds of situations have made you feel your emotions were beyond control?

23. What kind of judgment did you tend to exercise in problem situations?

Can you think of situations in which you believe that you used poor judgment or acted irresponsibly? Did you often repeat the same mistakes?

24. Did others think of you as selfish?

25. Describe circumstances in which you might lie or cheat.

26. Have you ever been in trouble with the law?

27. Describe your past and current use of alcohol, including estimate of amount consumed. If you have ever had a problem with drinking:

a. were you ever intoxicated before marriage?

b. Frequency of intoxication in the year before marriage?

c. Intoxicated after the wedding? How frequently?

28. Did you ever use drugs, even prescription drugs, on a regular basis?

a. What drugs did you use?

b. How frequently during courtship and engagement?

c. During marriage?

d. Effect of use of drugs?

29. Describe your past ability to handle money. Explain any creditor problems.

30. What recurring fears, if any, bothered you?

31. Thoughts/attempts at suicide?

32. Have you ever sought and received counseling? Have you ever been hospitalized for a nervous breakdown or emotional disorder?
